Population-attributable risk of psychiatric disorders for suicide among adolescents and young adults in Taiwan

Background
Youth suicide rates have increased markedly in some countries. This study aimed to estimate the population-attributable risk of psychiatric disorders associated with suicide among Taiwanese youth aged 10–24 years.
MethodsData were obtained from the National Death Registry and National Health Insurance (NHI) claims database between 2007 and 2019. Youth who died by suicide were included, and comparisons, 1:10 matched by age and sex, were randomly selected from the Registry for NHI beneficiaries. We used multivariable logistic regression to estimate suicide odds ratios for psychiatric disorders. The population-attributable fractions (PAF) were calculated for each psychiatric disorder.
ResultsA total of 2345 youth suicide and 23 450 comparisons were included. Overall, 44.8% of suicides had a psychiatric disorder, while only 7.9% of the comparisons had a psychiatric disorder. The combined PAF for all psychiatric disorders was 55.9%. The top three psychiatric conditions of the largest PAFs were major depressive disorder, dysthymia, and sleep disorder. In the analysis stratified by sex, the combined PAF was 45.5% for males and 69.2% for females. The PAF among young adults aged 20–24 years (57.0%) was higher than among adolescents aged 10–19 years (48.0%).
ConclusionsOur findings of high PAF from major depressive disorder, dysthymia, and sleep disorder to youth suicides suggest that youth suicide prevention that focuses on detecting and treating mental illness may usefully target these disorders.
Evaluation of bi-directional causal association between depression and cardiovascular diseases: a Mendelian randomization study

Background
Depression and cardiovascular disease (CVD) are associated with each other but their relationship remains unclear. We aim to determine whether genetic predisposition to depression are causally linked to CVD [including coronary artery disease (CAD), myocardial infarction (MI), stroke and atrial fibrillation (AF)].
MethodsUsing summary statistics from the largest genome-wide association studies (GWAS) or GWAS meta-analysis of depression (primary analysis: n = 500 199), broad depression (help-seeking behavior for problems with nerves, anxiety, tension or depression; secondary analysis: n = 322 580), CAD (n = 184 305), MI (n = 171 875), stroke (n = 446 696) and AF (n = 1 030 836), genetic correlation was tested between two depression phenotypes and CVD [MI, stroke and AF (not CAD as its correlation was previously confirmed)]. Causality was inferred between correlated traits by Mendelian Randomization analyses.
ResultsBoth depression phenotypes were genetically correlated with MI (depression: rG = 0.169; p = 9.03 × 10−9; broad depression: rG = 0.123; p = 1 × 10−4) and AF (depression: rG = 0.112; p = 7.80 × 10−6; broad depression: rG = 0.126; p = 3.62 × 10−6). Genetically doubling the odds of depression was causally associated with increased risk of CAD (OR = 1.099; 95% CI 1.031–1.170; p = 0.004) and MI (OR = 1.146; 95% CI 1.070–1.228; p = 1.05 × 10−4). Adjustment for blood lipid levels/smoking status attenuated the causality between depression and CAD/MI. Null causal association was observed for CVD on depression. A similar pattern of results was observed in the secondary analysis for broad depression.
ConclusionsGenetic predisposition to depression may have positive causal roles on CAD/MI. Genetic susceptibility to self-awareness of mood problems may be a strong causal risk factor of CAD/MI. Blood lipid levels and smoking may potentially mediate the causal pathway. Prevention and early diagnosis of depression are important in the management of CAD/MI.
Effect of antidepressants for cessation therapy in betel-quid use disorder: a randomised, double-blind, placebo-controlled trial

Aims
More than one-half of betel-quid (BQ) chewers have betel-quid use disorder (BUD). However, no medication has been approved. We performed a randomised clinical trial to test the efficacy of taking escitalopram and moclobemide antidepressants on betel-quid chewing cessation (BQ-CC) treatment.
MethodsWe enrolled 111 eligible male BUD patients. They were double-blinded, placebo-controlled and randomised into three treatment groups: escitalopram 10 mg/tab daily, moclobemide 150 mg/tab daily and placebo. Patients were followed-up every 2 weeks and the length of the trial was 8 weeks. The primary outcome was BQ-CC, defined as BUD patients who continuously stopped BQ use for ⩾6 weeks. The secondary outcomes were the frequency and amount of BQ intake, and two psychological rating scales. Several clinical adverse effects were measured during the 8-week treatment.
ResultsIntention-to-treat analysis shows that after 8 weeks, two (5.4%), 13 (34.2%) and 12 (33.3%) of BUD patients continuously quit BQ chewing for ⩾6 weeks among placebo, escitalopram, moclobemide groups, respectively. The adjusted proportion ratio of BQ-CC was 6.3 (95% CI 1.5–26.1) and 6.8 (95% CI 1.6–28.0) for BUD patients who used escitalopram and moclobemide, respectively, as compared with those who used placebo. BUD patients with escitalopram and moclobemide treatments both exhibited a significantly lower frequency and amount of BQ intake at the 8th week than those with placebo.
ConclusionsPrescribing a fixed dose of moclobemide and escitalopram to BUD patients over 8 weeks demonstrated treatment benefits to BQ-CC. Given a relatively small sample, this study provides preliminary evidence and requires replication in larger trials.
Utilizing additive manufacturing and gamified virtual simulation in the design of neuroprosthetics to improve pediatric outcomes

Additive manufacturing used with custom electromyographic sensors has been demonstrated for neuroprosthetic limb manufacturing and is now translating to the clinical environment. These manufacturing methods have dramatically reduced device weight while increasing the capability for multi-finger dexterity. Using wearable electromyography sensors standalone from the prosthetic limb, a new virtual training method has been designed and tested to improve human–machine interaction. This type of training leverages real-time visual feedback to user inputs, supporting improved timing and magnitudes of muscle contractions. The combination of these technologies may provide a stronger affinity between the pediatric patient group and the device.

Improved adhesion of diamondlike coatings using shallow carbon implantation

A surface layer of metal carbides provides an excellent interface to achieve a highly adherent diamondlike carbon (DLC) coating. A plasma immersion ion implantation (PIII)-based procedure is described, which delivers a high retained dose of implanted carbon at the surface of aluminum alloys. A shallow implantation profile, followed by argon sputter cleaning and continued until a saturated carbon matrix is brought to the surface, provides an excellent interface for subsequent growth of DLC. At a carbon retained dose above 1018 atoms/cm2 the DLC adhesion exceeds the coating's cohesion strength. Regardless of the silicon content in the aluminum, the coating produced by this method required tensile strengths typically exceeding 140 MPa to separate an epoxy-coated stud from the coating in a standard pull test. Improved DLC adhesion was also observed on chromium and titanium. The reported tensile strength is believed to substantially exceed performance of DLC coatings produced by any other method.
IUE Observations of WZ Sagittae in Outburst

The International Ultraviolet Explorer was used to obtain spectra at both 7 Å and 0.2 Å resolution of the recurrent nova WZ Sagittae during its 1978 outburst. The first spectra were obtained on 1978 Dec 1.8 UT, approximately 0.7 days after discovery. The decay from outburst was followed until 1979 Jan 1, after which it was too near the Sun to be observed. A post-outburst spectrum was obtained on 1979 July 11. In this paper, preliminary results from the analysis of the low resolution spectra are discussed. These observations will be used to support the similarity between WZ Sge and dwarf novae and to test accretion disk models.

In addition to offering observing time on a regular schedule, the International Ultraviolet Explorer (IUE) satellite is also being used to observe “targets of opportunity.” Novae represent one of the most exciting targets of opportunity and also one of the most difficult because of their rapid time behavior. During the first year of operation of IUE we were extremely fortunate to have three bright novae outbursts occur: Nova Cygni 1978, WZ Sagittae, and U Scorpii.
Nova Cyg 1978, a fast nova, reached a maximum apparent visual magnitude of 6.2 on September 12, 1978 (Slovak and Vogt 1979). Figure 1 shows the relative flux of Nova Cyg 1978 in the long ultraviolet wavelength range (1900-3200Å) at several different times. One day after maximum (Sept. 13), the nova is still in its absorption line phase. At this point it looks like a supergiant F star with mostly Fell absorption lines (also see Cassatella et al. 1979). Later spectra show the emergence of emission lines, which is the characteristic behavior of novae in the visual. Magnesium II at 2800Å is the strongest emission line in this part of the spectrum for at least a month after outburst. The last spectrum (Nov. 1) shows mainly semi-forbidden lines of carbon, nitrogen, and oxygen. It should also be noted that the slope of the continuum flattens as the nova evolves.
